6.58 What are the concept and treatment of ininbar intervertebral disc protrusion?

Lumbar intervertebral disc protrusion is a conthtion associated with the ruptured annulus fibrosus and results in the nucieus ptilposus bulging out to compress nerve roots or the cauda equina nerve. Thus, this lead to a series of symptoms. It is common in adults and clinically common is the protrusion of tile disc between the 4th and 5th urnbar vertebrae arid that between the 5th lumbar vertebrae and the I st sacra joint.

[Treatment]
The principle of treatment is as follows: dredging the channeh and collaterals, relieving spasm and pain, releasing the adhesion, and red icing compressing to the never roots. During the treatment period, the patient should rest on a hard bed uniii alLeviation of syrnptoms- appropriate physicai exercise of the lumbar muscles is necessary and very important.
(1 )Comrnon manual techniques
Gun-rolling, An-pressing. Rou-stationary circular pressing, Tui -pushing, Na-holding, Yao -rotation, Ba - tract ion., dou -shaking, Ban-manipulation, etc.
(2)Process
¢ÙWith the patient in prone1 stand at the affected side and use C-un-rolling, An-pressing, and Roy-stationary cireuar pressing to the hack waist and buttocks for about 20 minutes.
¢ÚApply Than- ffinnger pressing and An -pressing to Sherishu Yaowrzg-guan, Hucnitiao, Weizhong, Yanglingquan, Chengshan and Kunlun for 2-3 minutes.
¢Û Apply An -pressing and swing with hands to the waist and rhythmically repear for 2 - 3 minutes.
¢ÜLet the patient hold the bed, Hold both ankle joints and apply liflction. Swing for a while and shake once. The wa.st should be out oi the bed. Repeat 3 irnes.
¢ÝObligue Ban -manipulation.
¢ÞSitring-rotational-replaccinent.
¢ßAppIy Tui -pwhitig from back to sacrurn 10 times.
¢àWith the patient supine, . stand on the affected side . hip aginst the patient¡¯s thigh. with one hand hc4ding rhc heel, and the lower hand holding the forefoot . Passively raise the lower limb 10-15 times, then, bend ilw k-nec anti hip. Apply traction and shake the affected leg 3 -5 times. In thc acute stage, the patient must rest on a bard bed, and shodid be treaied with mild nianipulation( with serious attentbn to Ban -manipulation). If symptoms such as numbness of the seilar area, and incontinence of urine and stooI are present, manipulation is forbidden.
